Birth certificates must be sent with the necessary forms as found in the chart below. Thank you.
Day students must submit an application, health assessment, sports form (if participating in athletics), non aspirin permission form, emergency care information, authorization for administration of medication. These forms are found in the chart below.
Boarding students must submit all forms required by day students, as well as the weekend permissions form and the permission to visit other homes form (found in the chart below).
International students must submit all forms found in the chart.
